Back to School Checklist

OptionC is the web-based Student Information System (SIS) utilized by St. Veronica School to maintain student attendance and grades (K-8), send important school alerts to parents, and maintain required school forms.

All parents must complete required school forms each year by logging into the parent portal. eForms can be found under Communication/eForms. eForms will be available for completion beginning July 30, 2026 and should be completed prior to the first day of school.

Parents of new students will receive an email with OptionC login instructions.

All students in K-8 must have a Dismissal Plan, which will be maintained in the school office and with the homeroom teacher. Any changes to the dismissal plan requires a note sent in the morning. A phone call or email to Bonnie Ferber [email protected] will also be accepted, but the change must be made prior to 2:00 PM.

Print and complete one Dismissal Plan Form per student and return during the Back to School Block Party on August 14 or on the first day of school.

Preschool & Pre-K: Submit a new or updated (if expired) Child Medical Statement.

7th Grade: Submit Tdap and MCV4 Vaccination Form OR updated immunization record showing these vaccines.

All Students: Submit new immunization record as updates occur. Submit any applicable medication forms.

Turn in paper copies to the school office, submit electronically to [email protected], or fax to (513) 528-0513.

West Clermont | Complete the Parochial School Transportation Form and return it to Bonnie Ferber ([email protected]) as soon as possible.
Batavia | Complete the online Parochial Transportation Request.

Milford | Complete the Private/Parochial Out of District Schools Request Form and return it via mail, email, or fax to the district (details included on the form).

Check your school district's transportation Bus Route E-Link (West Clermont / Milford / Batavia) for busing information. The login for West Clermont's Bus Route E-Link is the student's first two letters of the last name + first four letters of the first name (student ID/username) and the student's 8-digit DOB (password). **Please note that West Clermont may not publish routing information until a couple of days before schools begins. Check back frequently.**

Check out the 2026-2027 Supply Lists. Supplies can be brought in during the Back to School Block Party on August 14 or on the first day of school. Please note, pre-k students use the preschool list.

If your family will utilize before- or after-school Latchkey, print and complete a Latchkey K-8 Registration Form and/or a Latchkey Preschool Registration Form and return during the Back to School Block Party or on the first day of school. Please note, billing is done through FACTS. Please do not send cash or check.

Throughout the school year, parent volunteers will be needed for various events, field trips, cafeteria help, etc. All volunteers must be certified through SafeParish. For information on how to complete this process, click here. For returning volunteers, please ensure you have completed all available training.

If you received a letter from the state with an EdChoice Acceptance Form, return your signed form to [email protected] before the first day of school. If you have not received this, complete a blank form found here. If you are a NEW applicant, you must also complete and return the Signature Authorization form. Each K-8 family will receive a Family Income Form during the second week of school. This information is very important to ensure our continued participation in federal programs. These programs provide a variety of materials and services for the students, teachers, and our school. It is one of the benefits our students receive from your tax dollars, and we do not want to lose it! Please return the Family Income Form to the school office by September 4th.
