Login to your OptionC account to complete all required electronic forms for the 2025-2026 school year. eForms can be found under the Communication tab. (New families should have received an email with login instructions.)
Print and complete the required Dismissal Plan (one form per K-8 student). Copies will also be available at the Back to School Open House on August 18th. This form MUST be returned on the first day of school, or turned in at the Open House.
Review the Supply List for your applicable grade(s). Supplies can be brought with you to the Preschool / Pre-K / Kindergarten Orientations on August 13th and 15th, the Back to School Open House on August 18th, or with your student on the first day of school.

Copies will also be available at the Preschool / Pre-K / Kindergarten Orientations on August 13th and 15th and the Back to School Open House on August 18th. This form should be returned on the first day of school, or turned in at Orientation or Open House. **Do not send payment. You will be invoiced via FACTS.**
Throughout the school year, parent volunteers will be needed for various events, field trips, cafeteria help, etc. All volunteers must be certified through SafeParish. For information on how to complete this process, click here.
If you received a letter from the state with an EdChoice Acceptance Form, return your signed form to [email protected] before the first day of school. If you have not received this, complete a blank form found here. If you are a NEW applicant, you must also complete and return the Signature Authorization form. Each K-8 family will receive a Family Income Form during the first week of school. This information is very important to ensure our continued participation in federal programs. These programs provide a variety of materials and services for the students, teachers, and our school. It is one of the benefits our students receive from your tax dollars, and we do not want to lose it! Please return the Family Income Form to the school office by September 1st. If you have not already done so, complete the transportation request as provided by your district of residence. Refer to the May 29th edition of The Viking Voice previously shared or found here.
Check your school district's transportation Bus Route E-Link (West Clermont / Milford / Batavia) for busing information. The login for West Clermont's Bus Route E-Link is the student's first two letters of the last name + first four letters of the first name (student ID/username) and the student's 8-digit DOB (password). **Please note that West Clermont may not publish routing information until a couple of days before schools begins. Check back frequently.**
